Not a member? Sick visits are $65 and school/sports physicals are $50, subject to availability.

Meet Kari Nelson, MSN, FNP-BC.
Kari is a board-certified Family Nurse Practitioner and a Cary resident. She's spent her career caring for patients in primary care, including at Christie Clinic, the VA, and Northwestern Medicine.
She started Cary Primary Care to build lasting relationships with patients and make room for thoughtful conversations, shared decisions, and care that fits your goals.

Your first appointment is about getting to know you.
We schedule 60 minutes for initial primary care appointments so there's time to review your history, talk through concerns, answer questions, and build a care plan together. You shouldn't feel rushed through your first visit.

After booking, we review the information you provide and contact you if anything needs clarification before your visit.

Complete membership setup
We will request the information needed to set up the appropriate membership and payment method before or around your first visit.

Once established, contact us for routine care, new concerns, follow-up, medication needs, and preventive care.

Routine & Chronic Care
Ongoing management of common primary-care conditions and medication follow-up.
Sick Visits
Included with membership at no extra visit charge. Non-members can book sick visits for $65, subject to availability.
School & Sports Physicals
Included for eligible members at no additional visit charge. Available to non-members for $50.

Clear monthly pricing.
Sick visits and school/sports physicals are included for members at no additional visit charge. We care for children ages 5 and up. Non-member visits are available separately.
Standard monthly rate
Adult Primary Care
$99/month
Ongoing adult membership.
Additional Child Membership
$25/month
For each eligible additional child with an enrolled parent or legal guardian. We see children ages 5 and up.
