Child Primary Care Membership · Ages 5–17
$49/month
Available for children ages 5–17 with an enrolled parent or legal guardian.
Add-On Weight Management
$49/month
Available as a recurring add-on for active primary care members.
Stand-Alone Weight Management
$119/month
Ongoing weight management for patients who are not enrolled in the primary care membership.
Eligible Household Maximum
$299/month
Maximum for eligible enrolled adults and children ages 5–17 living in the same household. Weight management services are separate.
How enrollment works. Start by booking the appropriate first appointment. Cary Primary Care will review your information, request payment details, and enroll you in the appropriate membership plan. Primary care membership billing begins when you establish care at your first visit.
What's included
Primary care without a separate charge for each included visit.
Annual preventive exam
A dedicated yearly assessment focused on screenings, risk factors, preventive planning, and health goals.
Routine primary care
Evaluation and management of appropriate routine concerns, ongoing conditions, and medication needs.
Priority acute visits
Same-day or next-day appointments when available for appropriate non-emergency concerns.
Telehealth
Virtual visits for concerns that can be safely evaluated without an in-person examination.
Chronic care
Ongoing management of common stable conditions when appropriate.
Care coordination
Referrals, outside-record review, laboratory follow-up, and help navigating specialist recommendations.
Insurance
Membership is not insurance.
Cary Primary Care does not bill insurance for services included in your membership. You may continue to use your health insurance for outside services whenever it is accepted by the outside provider or facility.
Patients should maintain appropriate coverage for services outside the practice, including emergencies and hospitalization.
Children & families
Simple family eligibility.
Child membership is available for ages 5–17 when a parent or legal guardian is also enrolled in primary care. At age 18, patients transition to an individual Adult Primary Care membership.
The eligible household membership maximum is $299 per month.
Outside services
What's not included in your membership?
Your membership covers primary care services provided by Cary Primary Care. Costs charged by outside providers, facilities, pharmacies, and laboratories are separate.
Medications & Testing
The cost of prescription medications, outside laboratory testing, X-rays, CT scans, MRIs, ultrasounds, and other diagnostic testing is not included.
Specialty & Facility Care
Specialist visits, urgent care or emergency department services outside Cary Primary Care, hospitalization, and procedures performed by outside providers or facilities are not included.
Other Third-Party Costs
Vaccines, medications, supplies, or other services that Cary Primary Care must purchase or that are provided by a third party may involve an additional cost unless specifically stated as included.
We'll still help coordinate your care. When outside testing or specialty care is needed, Cary Primary Care can help coordinate next steps and, when appropriate, send orders or referrals to the provider or facility you choose.
