Additional Child Membership
$25/month
For each eligible additional child with an enrolled parent or legal guardian. Pediatric primary care starts at age 5.
How enrollment works. Start by booking the appropriate first appointment. Cary Primary Care will review your information, request payment details, and enroll you in the appropriate membership plan. Primary care membership billing begins when you establish care at your first visit.
What's included
Sick visits and school/sports physicals included. No copays.
Members do not pay an additional visit fee for these services. Non-members may book sick visits and school/sports physicals separately, subject to availability. Non-member sick visits are $65; school/sports physicals are $50. In-office flu, COVID-19, strep, and urine testing are included with sick visits when clinically appropriate.
Annual preventive exam
A dedicated yearly assessment focused on screenings, risk factors, preventive planning, and health goals.
Routine primary care
Evaluation and management of appropriate routine concerns, ongoing conditions, and medication needs.
Sick visits included
Visits for appropriate common illnesses and acute concerns are included at no extra visit charge, with same-day or next-day availability when possible.
School & sports physicals included
School and sports physicals are included for eligible enrolled patients at no extra visit charge.
Telehealth
Virtual visits for concerns that can be safely evaluated without an in-person examination.
Chronic care
Ongoing management of common stable conditions when appropriate.
Lifestyle & weight management
Individualized support for nutrition, movement, sleep, and weight goals, with medication management when clinically appropriate, is included in primary care membership.
Care coordination
Referrals, outside-record review, laboratory follow-up, and help navigating specialist recommendations.
Insurance
Membership is not insurance.
Cary Primary Care does not bill insurance for services included in your membership. You may continue to use your health insurance for outside services whenever it is accepted by the outside provider or facility.
Patients should maintain appropriate coverage for services outside the practice, including emergencies and hospitalization.
Children & families
Simple family eligibility.
We see children ages 5 and up. Additional child membership is available for ages 5 through 20 when a parent or legal guardian is also enrolled in primary care. At age 21, patients transition to an individual Adult Primary Care membership.
We do not currently stock or administer vaccines in the office. We review immunization records, recommend appropriate vaccines, and can direct families to outside vaccination providers. We do not remove patients from our panel solely because they do not follow the recommended vaccine schedule.
Membership is $99 monthly per enrolled adult, plus $25 monthly for each additional eligible child. There is no family maximum.
Your care, your voice
Decisions made together.
We respect patient autonomy. We will explain our clinical recommendations and discuss the benefits, risks, and alternatives, but your goals and preferences matter. Care plans are developed with you, not imposed on you.
Outside services
What's not included in your membership?
Your membership covers primary care services provided by Cary Primary Care. Costs charged by outside providers, facilities, pharmacies, and laboratories are separate.
Medications & Testing
The cost of prescription medications, outside laboratory testing, X-rays, CT scans, MRIs, ultrasounds, and other diagnostic testing is not included.
Specialty & Facility Care
Specialist visits, urgent care or emergency department services outside Cary Primary Care, hospitalization, and procedures performed by outside providers or facilities are not included.
Other Third-Party Costs
Medications, supplies, and services provided by third parties may involve additional costs unless specifically stated as included. Vaccines are not currently offered in the office; see our children and families information above.
We'll still help coordinate your care. When outside testing or specialty care is needed, Cary Primary Care can help coordinate next steps and, when appropriate, send orders or referrals to the provider or facility you choose.
