Initial Evaluation
We review your weight history, previous weight-loss efforts, medical conditions, medications, nutrition, activity, sleep, and goals. Baseline laboratory testing may be recommended when clinically appropriate.

Medical weight management
Individualized clinical care for adults working toward meaningful, sustainable weight loss through medical evaluation, lifestyle planning, ongoing follow-up, and medication when clinically appropriate.
How it works
Weight management is not one-size-fits-all. Your treatment plan is based on your health history, weight history, goals, medications, lifestyle, and individual risk factors.
We review your weight history, previous weight-loss efforts, medical conditions, medications, nutrition, activity, sleep, and goals. Baseline laboratory testing may be recommended when clinically appropriate.
Your individualized plan may include calorie and nutrition targets, protein and activity goals, behavioral strategies, and prescription weight-management medication when medically appropriate.
FDA-approved weight-management medications, including GLP-1/GIP-based medications, may be considered when clinically appropriate. Enrollment does not guarantee a prescription or a specific medication.
Follow-up visits are used to review progress, side effects, treatment response, nutrition and activity, laboratory results, and whether your treatment plan should be adjusted.
What's included
Ongoing assessment of progress, treatment response, side effects, health changes, and appropriate laboratory results.
Selection, prescribing, dose adjustments, and monitoring of weight-management medications when clinically appropriate.
Practical recommendations for calories, protein, fiber, hydration, meal patterns, physical activity, and resistance training based on your needs.
Weight-management follow-up visits and treatment-plan adjustments are included while you are actively enrolled in the program.
When required for a clinically appropriate prescribed medication, we can submit prior authorization documentation to your insurance plan and provide reasonable appeal support when appropriate.
Treatment includes planning for continued progress, maintenance, and adjustment of your approach as your health and goals change.
Program pricing
$49/month
For enrolled Cary Primary Care members who want medical weight management added to their ongoing primary care.
$119/month
For adults who want medical weight management without enrolling in Cary Primary Care's primary care membership.
Prescription medications, laboratory testing, imaging, and other outside services are not included in the monthly program fee.
Insurance & prior authorizations
When prescription weight-management medication is clinically appropriate, you may use your insurance benefits or another available pharmacy payment option.
If your insurance plan requires prior authorization for a medication we prescribe, Cary Primary Care can submit the required clinical documentation and provide reasonable appeal support when appropriate.
Insurance plans determine their own coverage criteria, formularies, deductibles, copays, and exclusions. Some plans do not cover medications for weight management at all. Prior authorization support does not guarantee approval or payment by your insurance plan.
Medication costs and other pharmacy expenses remain the patient's responsibility.
Medication expectations
Medication may be one part of your treatment plan, but it is not appropriate for every patient. Prescribing decisions are based on your medical history, clinical evaluation, potential risks and benefits, treatment goals, and appropriate monitoring.
Enrollment in the program does not guarantee eligibility for a GLP-1 medication, insurance approval, or a prescription for any specific medication.
